Treatment options

Endodontic disease can be treated according to the degree of pulp damage. Generally, it is impossible to make an accurate histopathological diagnosis of pulp changes in clinical practice. Instead, two different treatment methods are selected based on clinical manifestations and clinical diagnosis: 1. Diagnosis of pulp lesions as localized or reversible, and selection of treatment methods aimed at preserving the vital pulp, such as direct pulp capping, indirect pulp capping, and pulpotomy. 2. If the pulp lesions are diagnosed as total or irreversible, choose a treatment method aimed at removing the pulp and preserving the affected tooth, such as root canal therapy, pulp plasticization therapy, etc. The treatment for periapical periodontitis can include root canal therapy.

Nursing Assessment

1. Check the health history of patients with acute apical periodontitis and ask whether they have a history of pulpitis.

2. Physical condition of patients with acute apical periodontitis. Acute apical periodontitis is mostly caused by an acute attack of chronic apical periodontitis. According to its development process, it can be divided into serous stage and suppurative stage. In the early stages of inflammation, the affected tooth will feel like it is floating and will be painful when chewing. The patient can point out the affected tooth and there will be percussion pain during examination. When purulent apical periodontitis forms, there will be throbbing pain and the affected tooth will have a noticeable stretching sensation. The submandibular lymph nodes will swell. If the condition worsens and the pain becomes severe, it may be accompanied by symptoms such as fever and general discomfort. When the abscess reaches the periosteum and submucosal, a sense of fluctuation can be felt (as shown in the right picture). Acute inflammation can be relieved after the abscess ruptures or is incised and drained.

3. Social and psychological factors: Due to the severe pain, patients often seek medical treatment in a timely manner and ask the doctor to resolve their pain as soon as possible.
